Thin endometrium is a clinically significant condition in infertility practice characterized by inadequate endometrial thickness (<7 mm) during the peri-ovulatory phase, which negatively affects embryo implantation and pregnancy outcomes. A receptive endometrium is essential for successful implantation, and optimal thickness is generally considered to be 7–12 mm with a trilaminar pattern on ultrasound. Thin endometrium may result from chronic endometritis, previous uterine curettage (Asherman’s syndrome), genital tuberculosis, poor uterine blood flow, hormonal insufficiency, or prolonged use of anti-estrogen drugs like clomiphene citrate. Clinically, patients often present with infertility, repeated implantation failure in IVF cycles, or history of scanty menstrual flow. Diagnosis is made by transvaginal ultrasound, which shows reduced endometrial thickness and sometimes poor vascularity on Doppler studies. Management is often challenging and requires a multimodal approach, including estrogen supplementation, improving uterine blood flow (aspirin, sildenafil), use of granulocyte colony-stimulating factor (G-CSF), platelet-rich plasma (PRP) therapy, and treatment of underlying infections such as tuberculosis or endometritis. In assisted reproductive techniques, optimizing endometrial receptivity is crucial, and strategies such as freeze-all cycles with later embryo transfer after endometrial preparation may be employed. Early identification and targeted management can significantly improve implantation rates and IVF success outcomes.
