Ovulation induction is a key fertility treatment aimed at stimulating the ovaries to develop and release one or more mature follicles, thereby improving the chances of conception in women with anovulation or oligo-ovulation. It is commonly used in conditions such as Polycystic Ovary Syndrome (PCOS), unexplained infertility, luteal phase defects, and mild male factor infertility (in combination with IUI). The process involves the use of oral or injectable medications to promote follicular growth, with the most commonly used drugs being letrozole (first-line), clomiphene citrate, and gonadotropins, depending on patient profile and response. Letrozole is preferred due to its better ovulation rates, monofollicular development, and lower risk of multiple pregnancy compared to clomiphene. The cycle is monitored with serial transvaginal ultrasound (follicular monitoring) to assess follicle size, number, and endometrial thickness, ensuring optimal timing for ovulation. Once the dominant follicle reaches an adequate size (usually 18–20 mm), an ovulation trigger (hCG or GnRH agonist) is administered to induce follicular rupture, followed by timed intercourse or intrauterine insemination (IUI). Careful monitoring is essential to avoid complications such as multiple pregnancy and ovarian hyperstimulation syndrome (OHSS). Ovulation induction plays a central role in modern infertility management and, when properly individualized, significantly improves conception rates and treatment success, including better outcomes in IVF protocols.
